Gangrenous Adult Midgut Volvulus from Midgut Malrotation: A Case Report

چکیده
Malrotation of the midgut is mainly a childhood congenital anomaly. Adult Midgut malrotation is rare with the clinical symptomatology varied and non-specific making pre-operative diagnosis difficult. Midgut volvulus from malrotation is a true surgical emergency requiring early intervention to prevent bowel necrosis and death. We report a 27yr old male who was referred to our centre with paralytic ileus post appendectomy. He underwent emergency laparotomy with findings of gangrenous volvulus involving almost the entire midgut from malrotation. He had bowel resection with jejunocolic anastomosis. Post-operatively he was referred on account of short bowel.A high index of suspicion is needed in acute abdomen with atypical features to allow for early diagnosis and prompt intervention to avert this catastrophic event.

Imaging Findings of Midgut Volvulus Associated with a Large Small-Bowel Diverticulum in an Adult Patient: Case Report
abdomenal distress in adults (1). Midgut volvulus is mostly related to intestinal malrotation, and it is rarely associated with postoperative adhesions and a bulky diet after fasting (2). Although acquired jejunoileal diverticula are uncommon and asymptomatic in the majority of patients, the association of midgut volvulus and a large small-bowel diverticulum has been mentioned only in a radiolo...

متن کاملUtility of the CT Scan in Diagnosing Midgut Volvulus in Patients with Chronic Abdominal Pain
Symptomatic intestinal malrotation first presenting in the adults is rare. Midgut volvulus is the most common complication of malrotation in the adults. Because of more differential diagnosis, Computed Tomography (CT) scan can play an important role in the evaluation of patients with this abnormality. The whirl pattern around the superior mesenteric artery found on CT scan in patients with midg...

Congenital midgut malrotation is a complex gastrointestinal anomaly, which could easily lead to midgut volvulus and gastrointestinal obstruction. Large studies on congenital midgut malrotation in adults are rarely investigated. The current study aimed to explore the clinical profile and diagnostic modalities of congenital midgut malrotation in Chinese adult patients. Clinical and radiological d...

We report a case of a 53-year-old male who presented with acute onset right upper quadrant (RUQ) abdominal pain with investigations demonstrating malrotation causing midgut volvulus and subsequent intestinal obstruction. The patient was consented for an emergent operation and underwent an exploratory laparotomy. Operative findings included the presence of adhesive bands in the RUQ (Ladd's Band)...
